All screenings will take place at the site from 9:00 am through 12:30 pm. We will screen the children by time of arrival and area of concern. This is a screening for children who are not presently attending the Boston Public Schools and are residents of the City of Boston. The location will be the first floor. The child being screened needs to attend the screening with their parent/guardian. Please bring proofs of residency to the screening. The www.bostonpublicschools.org website will list the documents needed. We will not need the immunizations for this process.
Please call or email Maryann Molloy to RSVP for the screening date you are interested in attending with your child. The phone contact is 617-635-8599 and email is email@example.com
Translated documents are available. Please contact Maryann Molloy.
Proofs of residency will be needed in case we go forward for a formal assessment. Please see backside for documents needed.
