The Office of Human Capital has transitioned to using online forms. Applications for reassignment, job share, additional program area, and change in FTE status can now be found at the links below. Please note that employees will be required to sign in with their Boston Public Schools Gmail account. Some supplemental materials may need to be submitted—these are noted in the form itself and below. These links will also be made available through BTU and Paraprofessional representatives, the Boston Public Schools website, and the Superintendent’s bulletin.

ELAR License Information must be emailed to firstname.lastname@example.org in addition to filling out the form above.
To download your License Information, log into your DESE account, click the link under Educator Services for the ELAR Welcome Page, and click on "Check license status and history, make a payment". At the bottom of the page under License Information, click "View/Print".
This application is only available to permanent teachers, permanent school nurses, permanent student support services coordinators, and paraprofessionals.
All employees impacted by a reduction in FTE must submit this form acknowledging their understanding.
Each applicant must submit one form. The principal/headmaster and the Office of Human Capital must approve requests.
All educators and other employees seeking licensure related verification and/or loan forgiveness must complete this form. Please direct any questions, comments, inquiries, or supporting documents to email@example.com.

Principals must fill out this form in order to approve job share requests for their building.
Principals of Pilot, Innovation, Turnaround, and In-District Charter Schools must fill out this form if they are excessing permanent teachers.
Principals of traditional schools should complete this form to inform central office budget forecasting if they intend to open-post positions in the upcoming hiring cycle.
