Additional New Hire Forms and Checklists

The following additional documents must be completed and presented to the Office of Human Resources(OHR). For help completing any of the forms, please call 617-635-9600.

Additional Documents which must be completed and submitted to OHR:

• Valid Massachusetts Driver License for review
• Copy of a High School Diploma or G.E.D
• Official Transcripts for Undergraduate and Graduate work completed
• Documentation to support passing score on the Boston Police, Rule 400, Special Officers exam
• Documentation to support passing an extensive medication. (Done at your own expense)
• Documentation to support passing a physical fitness standards test
• Residency Affidavit
