Schedule time with The New Teacher Support Team to bring in your completed forms and the following items:
  1. Social Security Card (Info on Getting a Replacement SSN Card)
  2. Voided check (if needed for Direct Deposit Forms)
  3. Official transcripts for undergraduate and any graduate work completed
    Undergraduate transcripts must state that the degree was conferred and include degree conferral date.
    Candidates with educational backgrounds from abroad (excluding Puerto Rico, Guam, U.S. Virgin Islands or its territories) will need to contact the Center for Educational Documentation (617-522-4738), which provides professional assistance in evaluating the educational background of persons educated abroad. They can interpret from a number of languages (Spanish, Portuguese, German, Finnish, Swedish) for an additional $50. For direct translation, contact the International Documentation Center at 1-877-777-ASIA or www.eurasiacom.com.
  4. Massachusetts Teacher’s License (Information on Licensure is available in the Teachers Resources section under Licensure)
    We must see the original copy.  If you do not yet have a license in Massachusetts, see the Licensure page.
  5. If applicable, a letter verifying previous full-time teaching experience (see sample)
    For upgrades in salary, only full-time teaching experience as a licensed teacher will count.  Letters from private schools or public school districts on official letterhead are acceptable for verification.  Letters must be an original document on letterhead, verifying at least one hundred sixty (160) days under contract performed under a license granted by the Mass. Department of Education or certification that requires equal qualification. Please include your dates of employment.
